知情同意说明 / Consent and Acknowledgement:

1. 活动收款与盈余声明 / Activity Collection and Surplus Fund Declaration:

我了解并接受慈济基金会是非营利组织，致力推动慈善公益活动。活动的收款主要用于支付直接性开销，盈余资金不会分配给任何成员／志工。我特此同意将活动余额捐入【慈济发展基金】，以支持日常运作开销之用途。I acknowledge and accept that Yayasan is established as a not-for-profit organization dedicated to carrying out charitable activities. As such, no surplus funds will be distributed to any of its members or volunteers. All funds collected will be used solely to cover the costs and expenses incurred in the course of its operations. I hereby consent to any remaining balance being donated to Yayasan’s development fund to support and advance its charitable mission. 

2. 个人资料保护声明 / Personal Data Protection:

所有个人资料将予以保密，仅限于马来西亚佛教慈济基金会（以下简称“本会”）内部使用。上述资料仅供本会在《马来西亚个人资料保护法令》规定下的适当用途范围内使用。任何为本会制作或属于本会的照片、影音录制（声音或影像）及文字资料，皆可由本会依其目的酌情使用。本人在此同意本会收集本人之个人资料，用于上述所列之用途。All personal information will be kept confidential and shared only within the Taiwan Buddhist Tzu Chi Foundation Malaysia ("the Foundation"). This information will be used exclusively by the Foundation for purposes deemed appropriate under the Malaysian Personal Data Protection Act. Any photos, audio recordings, video recordings, and written materials created for or owned by the Foundation may be used at the Foundation's discretion for its purposes. I hereby consent to the Foundation collecting my personal information for the purposes stated above. For more details, please refer to the Foundation's Privacy Notice.

每一梯次为期两个月的报名人数有限，若额满，则顺延下一梯次，敬请知悉。Please note that registration for each three-month intake is limited. If an intake is full, your registration will be carried over to the next available intake.
